USBA Western Region Qualifier
for USBA National Championship

Where: Burbank Elks Lodge
2232 N. Hollywood Way, Burbank, CA 91505
(1/4 mile south of Burbank [Bob Hope] Airport, across from Frys)

When: Dec 7 – Dec 9 (Fri-Sun)

Entry: $150 (includes $5 USBA sanction fee)

Nationals: Qualifiers have Entry Fee paid for Nationals (Louisville, KY, Jan 9 – 13)

Prizes: Cash prizes for top 3 – 6 finishers (depending on number of players);
if 20 players, and one goes to Nationals, prize fund is over $2,300

Who: All players must be members of the USBA (can join at the tournament),
AND must be U.S. citizens or permanent legal residents

Format: Determined by the number of entries (most likely flights of 6 – 8,
followed by a Finals flight 0f 4 – 6)

Matches: Prelims – 25 points; Finals – 35 points

Tables: 5 recently covered Verhoeven Paramount tables

Food:Complimentary morning refreshments and late-afternoon buffets

Deadline: Call or e-mail by 6:00pm, Thursday, Dec 6; NO WALK-INS

Fee/Dues: Bring $150 Entry PLUS USBA dues if necessary (if unsure, please call!)

Payment: Cash, certified checks or money orders ONLY – NO PERSONAL CHECKS
Payment MUST be made before 9:30am, Dec 7
